| If a physical therapist (PT) or occupational therapist (OT) gets a referral for a physical capacity evaluation (PCE) or work capacity
evaluation (WCE) from the treating physician, the PT or OT does not need to be on the director's list of authorized providers. |

| If the insurer
initiates the PCE or WCE, the insurer must use the director's list to select the physical
therapist or occupational therapist to conduct the exam. If the treating physician asks
the insurer to arrange the exam, the insurer does not have to use the director's list. |

| To be on the director's list, you need to complete the 3 hour training requirement in OAR 436-010-00265. There are some options for
obtaining the training. Our vendors have been approved to provide classroom training as well as training by DVD using a proctor or an on-line test, click
